By sharing knowledge, data, and resources, we accelerate solutions to complex challenges like homelessness, unemployment, and behavioral health. This deep level of collaboration allows us to address root causes, not just symptoms.
We currently maintain formal linkage agreements with over 100 community-based organizations, healthcare providers, government agencies, and social service institutions. These agreements allow us to refer individuals and families to trusted partners and follow up to ensure appropriate services were received—closing the loop with care and accountability.
