About this program

This program provides case management and supportive services to prevent the imminent loss of a Veteran’s home or identify a new, more suitable housing situation for the veteran or his or her family; or to rapidly re-house veterans and their families who are experiencing homelessness and might remain homeless without this assistance.

The program’s goal is to help participants to exit to a permanent housing destination, to maintain or increase income and/or employment, and to be connected to veteran benefits.
