Center for Integrated Health Solutions Selects A Safe Haven Foundation as Grant Recipient for Population Health Management in Behavioral Health

Chicago, IL, January 16, 2015 – A Safe Haven’s is one of four Illinois community-based organizations selected to participate in SAMHSA/HRSA’s Population Management Innovation Communities Initiative which aims to engage organizations in acquiring the knowledge and skills needed to implement measureable improvements in a high priority area related to healthcare integration. Lessons learned over the course of the Innovation Communities will be compiled and shared with the healthcare field so other organizations can benefit.

‘We are very excited to be involved in this innovative initiative,” said A Safe Haven President and Co-Founder, Neli Vazquez Rowland. “A Safe Haven looks forward to sharing and disseminating the important data and the lessons learned so that we may utilize the information to make more informed decisions in our behavioral healthcare system that allows us all to improve our efficiencies and outcomes.”

A Safe Haven’s Behavioral Health team will focus on topics and approaches that align with the following: Identifying applicability across integrated care settings; Addressing challenging problems related to integrated care; Establishing practical and meaningful performance indicators; Monitoring progress, implementation barriers, and effective strategies while identifying the tools and resources associated with successful implementation; and recording the lessons learned about the systemic and organization specific factors affecting the adoption & sustainability of integrated health innovations.
