Case Manager – Day for Change Program

This is a grant funded position

The Case Manager position is charged with coordinating and/or providing services in the following areas:  rehabilitation, housing, education, employment, and/or social work, including support for families.  The ideal candidate must have previous experience working with a wide variety of populations.  Duties include but are not limited to the following:

  • Participate in the intake process for new clients using the AWARDS database system
  • Effectively write case notes, letters, reports, maintain documentation, admission notes and complete required forms
  • Maintain records and case files, including individuals’ personal information, services provided, and relevant correspondence
  • Refer client to resources as needed to meet the objectives of their individual service plans
  • Provide assistance to clients as designated in the individual service plan
  • Effectively de-escalate and resolve client conflicts
  • Prepare and present a daily shift report designed to summarize encounters with clients, services provided and at includes all tasks performs as well as any issues that arise during a shift
  • Analyze information pertaining to individual service plans and clients progress



  • Knowledge of principles and processes for providing client services
  • Ability to make sound judgment calls
  • Effectively communicate with staff and residents of the organization
  • Ability to communicate information and ideas in writing so others will understand
  • Ability to maintain a positive attitude during difficult and high stress times
  • Knowledgeable about community programs and resources for clients
  • Ability to talk to others to convey information clearly and effectively
  • Must have valid driving license



  • Bachelor’s degree preferred. Associate’s degree and two to three years related experience required. CADC is a plus
  • Proficiency in Microsoft Word, Microsoft Excel, E-mail is also required
  • Valid Driver’s License Required
  • Bilingual English/Spanish a plus
